: Patients provided consent at intake at Maimonides Sleep Arts & Sciences (MSAS) to use their information anonymously for research purposes. All data were de-identified for this case series. The Los Alamos Medical Center Institutional Review board found the chart review exempt.

: Dr. Krakow operates multiple websites, offering products and services for sleep patients, such as:www.sleeptreatment.com; www.nightmaretreatment.com. He is medical director for Classic SleepCare, a national DME, and his sole function is QA. He has written 3 sleep books: Insomnia Cures; Turning Nightmares into Dreams; Sound Sleep, Sound Mind. He owns the commercial sleep center, Maimonides Sleep Arts & Sciences, Ltd. He has provided educational and consulting services for medical and mental health providers with occasional funding by vendors such as Respironics and ResMed or military institutions such as AMEDDC&S and VAMC. He has consulted for ASOCorp, a medical supply company. His nonprofit research center, Sleep & Human Health Institute, provides consultation services or receives grants for research studies, most recently funded byResMed Sciences $420,000 January 2015 through December 2019 (RCT of ASV vs CPAP in complex insomnia patients).
