: Ethical clearance to carry out this study was obtained from Kenya Medical Research Institute Scientific and Ethical Unit (KEMRI-SERU) and Walter Reed Army Institute of Research (WRAIR) institutional review boards (IRBs) and designated KEMRI/SERU/CCR/0167/4000 and WRAIR#2728 respectively.
