: Ethical approval for the study was obtained from the Ethical Review Board of Arbaminch University, Sawla campus [R/SC/843/31/2013]. Permission to undertake the research was obtained from every relevant authority in the Demba Gofa district. All participants were informed about the voluntary and anonymous nature of the study and were told that they could terminate their participation at any time during the survey: thus, informed consent was obtained.
