Ethical clearance was obtained from the Institutional Review Board (IRB) of the University of Gondar, College of Medicine and Health Science, Institute of Public Health. The reference number for the ethical clearance for this paper was IPH/1484/2013. Supportive letters were obtained from the local focal person of Metu district. Each study participant was informed about the purpose, method, expected benefit, and risk of the study.
