: The study was carried out according to the guidelines and regulations laid down in the declaration of Helsinki. The DHS Program uses protocols reviewed and approved by the ICF Institutional Review Board (IRB). In addition, IRBs in study countries review country-specific DHS survey protocols to make sure they comply with local laws on the inclusion of respondents, including minors and adults, in its surveys. In accordance with the ethics of conducting research with human subjects, voluntary consent to participate in the survey was sought from respondents after a clear description of the study, procedure, duration, possible risks and benefits of the study was explained for each study respondent.. Written informed consent was obtained from all study participants. IPUMS DHS granted access to the data.
