: Ethical approval for the study was obtained from the Health Research and Ethics Committee of the University of Nigeria Teaching Hospital Ituku/Ozalla, Enugu, with approval number NHREC/05/01/2008B-FWA00002458- IRB00002323. All methods were conducted according to relevant guidelines and regulations. A written permission was obtained from the Enugu State Universal Basic Education Board (ESUBEB). Informed written consent was obtained from the participating school headmasters/mistresses. Participation was voluntary and interviews were conducted with utmost privacy. Confidentiality was ensured by using codes instead of names on the forms and interview recordings.
