: The UK National Research Ethics Committee, Yorkshire and the Humber-South Yorkshire, approved the study on 4 February 2014 (reference []/YH/0019). We adhered to ethical guidance from the Good Clinical Practice Guide and the 1996 version of the Declaration of Helsinki. Written informed consent was obtained from every participant. Participant information and consent forms were co-written by our service-user and carer representatives.
