Ethics approval and consent to participate
: The study was conducted in accordance with the guidelines laid down by the Declaration of Helsinki and was approved by the Regional Ethics Committee of Western Norway (approval No.: 2015/75). All participants gave their written informed consent and were explained that they were free to withdraw from the study at any timepoint during the intervention without having to give a reason and without negative consequences for future care.
