Ethics approval and consent to participate
: The study was granted ethical approval by the UK National Research Ethics Service - North West - Liverpool Central Committee (16/NW/0791). Eligible participants were provided with a patient information sheet explaining the nature and the aims of the study. Participants had the opportunity to fully consider their participation in the study for at least 48 h. Signed informed consent were returned with the baseline questionnaires by post. All participants were allocated a study number to protect their anonymity.
