: Research ethics approval was granted by North West—Greater Manchester East Research Ethics Committee (reference 18/NW/0209) on June 14, 2018, and subsequently by the Health Research Authority on June 15, 2018. We are obtaining written informed consent from all participants in accordance with this approval. One substantial amendment to the protocol was made and approved on September 10, 2018, which is prior to the recruitment of the first participant.
