Ethics approval and consent to participate

: The Monash University Human Research Ethics committee (CF16/1920 - 2016000979) approved the study, and all participants provided written informed consent before data collection began. A post-recruitment amendment was submitted to the Monash University Human Research Ethics Committee (#7619) to approve the collection of de-identified variables from the AuSCR on non-responders (8 March 2018).
