Ethics approval and consent to participate
: The data collection has obtained authorisation from the University of Alberta ethics. This study was approved by the Human Research Ethics Board of the University of Alberta (Pro00085118). When registered by the clinician, the patient receives an email with a link to a patient-reported survey. Written information about the study, data protection and participants’ rights are in the survey with a request to confirm consent for using data for research purposes.
