: The East of Scotland Research Ethics Committee provided ethical approval for the trial (REC reference: 17/ES/0096). The study obtained ethics approval from NHS Scotland East of Scotland Research Ethics Service. Health Research Authority approval was not required for this study. Approval from Health and Care Research Wales was obtained. Management permissions were obtained from NHS Tayside Research and Development and NHS Wales Cardiff and Vale University Health Board Research and Development.
