Ethics approval and consent to participate

: The study protocol was approved by the Institutional Review Board (IRB) of the College of Health Sciences, Addis Ababa University meeting number 004/2015 and an approval letter was written on the date of May 6,2015with protocol number 008/15/SPH. Letter of permission was obtained from Amhara Regional Health Bureau and Bahir-Dar City Administration Zonal Health Office. Both ANC clients and providers were informed about the purpose of the study and verbal informed consent was also obtained before data collection. Study participants had the right to refuse participation or terminate their involvement at any point during observation. The information collected is kept confidential. Furthermore, report writing does not refer a specific respondent with identifiers.
