: Before starting the study, permission was given to download and utilize the data from . The Institution Review Board of ICF International’s DHS Program granted ethical approval. The Institution Review Board gave its approval to the DHS public-use data set procedures. Names of persons or household addresses were not included in the data files, and identifiers for respondents, households, or sample communities were prohibited in any manner. There are no labels on the data file that list the names or locations of any of the EA numbers. Since the study’s data set was made available to the public, neither patients nor members of the public were participating.
