Ethics approval and consent to participate
: This study received the necessary ethics and regulatory approvals from both Uganda and United Kingdom. The protocol endorsed by the steering group was approved by the Cure Children’s Hospital of Uganda Research ethics committee (Ref: CCHU-REC/09/019) and Uganda National Council for Science and Technology (Ref: HS444ES) in Uganda. The study was also approved by University of Liverpool Ethics and sponsorship committees (Refs: UoL001497–5573 and 5601).
