: Moffitt Cancer Center served as the coordinating institution for this multi-site study. The study protocol was approved by the Moffitt Cancer Center Institutional Review Board (IRB) of Record, Advarra (Pro00078625) including the informed consent documents for patient, caregiver, and healthcare provider participants. UNC ceded review to the Advarra IRB under a reliance agreement in accordance with DoD single IRB policy. Patient participants will provide written consent for study participation. Healthcare providers and caregivers who are participating in qualitative data collection will provide verbal consent for study participation.
