: The host university’s research governance team and the host NHS provider’s quality improvement and assurance team classified this study as a National Evaluation Audit (project number 4945). This meant it was exempt from ethical approval. However, we followed core ethical principles: obtaining written, informed consent from potential research participants that their (anonymised) responses would be used for research purposes, their participation was voluntary, and participants had the right to withdraw at any time.
