: This study complied with the Declaration of Helsinki (seventh revision, 2013) on medical protocol and ethics. This was a cross-sectional observational study. Since we collected the data from physicians’ charts filled out by the patients, the institutional review boards at each hospital waived the requirement of written informed consent but requested patients be given the opportunity to refuse enrollment by leaflets or the hospital website.
