: Ethical approval was obtained from the Far Eastern University-Nicanor Reyes Medical Foundation Institutional Ethics Review Committee (reference number: 2017-0056), Manila, Philippines, and Epidemiological Research Ethics Review Committee University of the Ryukyus (reference number: No. 382), Okinawa, Japan. Written parental consent forms were given to the parents of the potential participants aged 12-under 15 prior to the conduct of the study. In addition, the study participants were asked to sign a simplified assent form. Meanwhile, potential participants aged 15 to under 18 were asked to sign on the same informed consent document signed by the parents, as indicated in the specific guidelines for assent of the National Ethical Guidelines for Health Research, prepared by the Philippine Health Research Ethics Board Ad Hoc Committee for the Revision of the Ethical Guidelines [].
