: Ethical approval for this study was obtained from the Kenya Medical Research Institute (KEMRI) Scientific Ethical Review Unit (SERU) (KEMRI/SERU/3746 for the first phase and KEMRI/SERU/7/3/1 for the second phase) and the Institutional Review Board of the Institute of Tropical Medicine, Nagasaki University (140117120 for the first phase and 200910246 for the second phase).
