: The study was approved by the external reviewers and the Institutional Review Board of icddr,b, comprising the Research Review Committee and Ethical Review Committee (protocol no. PR-14038). We also obtained informed written consent from all participants after we described the study aim/objectives, purpose and procedures, and the participant risks and potential benefits, privacy, anonymity, confidentiality, and right not to participate and withdraw. We used the consent-taking process template of icddr,b. One copy of the signed informed consent form was provided to all participants. If any questions arise, CHW or investigator answered those questions for themselves and the family. If a woman is not certain whether she would like to participate, she was allowed to consider participation, and CHW returned after one week to revisit participation and answer any additional questions.
