: Ethical approval was obtained from the College of Medicine Research and Ethics committee in Malawi; the committee reference number is P.O8/17/2244. Informed written consent was obtained from the study participants. The participants indicated their willingness to participate in the study by signing or placing their thumb print on the informed consent sheet. Anonymity and confidentiality were ensured by using code names in the data collection materials. Privacy was ensured by conducting the interviews in a private place.
