Ethics approval and consent to participate

: The Ethics Committee of Royal Holloway, University of London, approved the MacTSQ design interviews. A UK National Health Service Research Ethics Committee approved the IVAN trial (07/NIR03/37). All procedures performed in studies involving human participants were in accordance with the ethical standards of the institutional and the national research committees and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards.

: Clare Bradley (CB) is the copyright owner of the MacTSQ, RetTSQ, DTSQ, ThyTSQ, RTSQ and other questionnaires which are licensed by Health Psychology Research Ltd. (HPR Ltd) for others to use. CB is a director and majority shareholder in HPR Ltd. and receives royalties when her questionnaires are licensed to commercial companies.
