: Collection of data from patients reported in this paper were approved by the Norwegian Data Protection Official for Research (10/01172–2), the Norwegian Regional Committees for Medical Research Ethics, south-eastern region B; 2017/1536, and the Oslo University Hospital Data Protection Official; 2017/15842. All patient participants signed a written informed consent.The general population data was collected in a parallel postal and web-based survey of the Norwegian population between May 10th and June 22nd 2010. The study was designed to provide a representative sample of the adult Norwegian population in terms of age, sex, and educational level. The study was considered outside the jurisdiction of the Regional Committee for Medical Health Research Ethics, and ethical approval was obtained from the Data Protection Official at Akershus University Hospital April 27th 2010.
