: The study was conducted in accordance with ethical principles of the Declaration of Helsinki. The study was approved by North Denmark Region Committee on Health Research Ethics (N-20,130,064), Danish Health and Medicines Authority (No. 2,013,092,582) and Danish Data Protection Agency (No. 2008-58-0028). The trial was approved in each participating country by the relevant ethics committees and drug agencies. EudraCT number 2013-003789-15 and ClinicalTrials.gov number NCT02572492. EORTC and FACT provided permission for use of the questionnaires.Every participant gave informed consent to participation in the trial before inclusion.
