: The study protocol was approved by the Institutional Ethics Committee, GMERS Medical College, Gandhinagar, approval letter no: GMERSMCG/IEC/13/2017 dated 1 March 2017.All the participants were given a clear explanation about the nature and purpose of the study in the language they understood, and written informed consent was obtained before enrolling the participants for the study. In the case of patients less than 18 years, assent of the minor was also obtained in addition to the informed consent from the parent or legal guardian.
