: The study has been performed in accordance with the Declaration of Helsinki and was approved by the ethics committee (Ethik-Kommission der Medizinischen Fakultät der Otto-von-Guericke-Universität, Leipziger Straße 44, 39,120 Magdeburg) of the Medical Faculty of the Otto-von-Guericke-University Magdeburg under the reference number 71/99. All participants provided written informed consent. For the participating children written informed consent was obtained from their parents/legal guardians.
