Ethics approval and consent to participate

: Written informed consent was given by all patients before inclusion in the DREAM cohorts.The Medical Ethics Committees of the Medisch Spectrum Twente, Enschede and Isala, Zwolle hospitals determined, in accordance with Dutch Law on medical-scientific research with humans, that no ethical approval was required because all data were collected in the course of regular daily clinical practice. Nonetheless, patients were completely informed and written informed consent was obtained from each patient.
