: Ethics approval for this observational study was obtained from the Ethics Commission of the Medical Department of Goethe University, Frankfurt am Main, Germany (No. 122/09). The study was performed in compliance with the ethical principles of good clinical practice and according to the International Conference on Harmonisation Harmonised Tripartite Guideline for Good Clinical Practice. All patients were informed of the objectives of the observational study and gave written consent for their voluntary participation in the study and the anonymous use of personal data in statistical analyses.
