: Study protocol was exempted from ethics review by the medical ethical committee (METC Oost-Nederland, the Netherlands; CMO number 2019–5459). Eligible patients were approached by sending them a patient information letter and a questionnaire. Informed consent was obtained from all participants involved in the study by signing and dating the questionnaire, and only then data were gathered.
