: This study was performed in line with the principles of the Declaration of Helsinki. Approval was granted by the ethical committee (EC) of Alexandria University Faculty of Medicine (protocol approval serial number: 0305296) which functions according to the International Conference of Harmonization Good Clinical Practice (ICH GCP 2011) and applicable local and institutional regulations and guidelines. The EC has a federal-wide assurance (FWA) since 2010. A written informed consent was obtained from all subjects and in the case of minors their parents or their legal guardian before enrolment in the study. Each participant was given a code to assure confidentiality. All methods were carried out following relevant guidelines and regulations.
