: The co-authors declare no conflicts of interest. The organization and implementation of the consensus conference was carried out by the Americas Health Foundation (AHF), a 501(c)3 nonprofit organization dedicated to improving healthcare throughout the Latin American Region and was supported by unrestricted grants from Roche. The AHF was responsible for the development, organization and implementation of the consensus conference, along with independently selecting the experts to serve on the panel. The AHF had no role deciding the content of the manuscript and the recommendations are those solely of the panel members.
