: Patient provided oral consent.Authors were unable to get written consent because medically unnecessary, face-to-face clinic appointments were prohibited due to the global COVID-19 pandemic. At the time of submission of this report, patient was no longer followed in our clinic because he was changed to a different anticoagulant that does not require routine lab monitoring.This report poses no more than minimum risk. Risk of confidentiality breach is minimized because all patient data has been de-identified.
