Ethics approval and consent to participate
: The ethical approval letter was obtained from Medical Research Ethics Committee (Formerly known as Medical Ethics Committee),University of Malaya Medical Center (MRECID.NO: 20181031-6826) on 15 January 2019. Data were collected using the structured questionnaires. Each questionnaire was put in an envelope which contains a cover letter, information sheet regarding the purpose of research, and contact information. Participants were informed about steps to be taken and the voluntary of participations to maintain confidentiality. The returned and completed questionnaires mean consent to participate.
