: We obtained oral consent from all individual respondents included in the study. Oral consent was used due to low-literacy levels in our study population and because the consent document would have provided the only link between participants and their responses. Nigeria Health Research Ethics Committee (#NHREC/01/01/2007–18/05/ 2017), and institutional review boards at the University of Wisconsin - Madison (#SE-2011-0859), Hunter College of the City University of New York (#2016–0352), and The Pennsylvania State University (#00011190) approved all research activities included in this study.
