: This study was approved by the Institutional Review Boards of the VA NY Harbor Healthcare System and NYU Langone Health. The study was granted a waiver of documentation of consent for providers to participate in the implementation interventions (training, clinical reminders) and the surveys. The study was granted a waiver of documentation of consent for patient participation in the phone surveys. Patients and providers will provide written informed consent to participate in the interviews.
