: The study protocols for the pilot studies and main trial by Community Empowerment Lab (CEL) Ethics Review Committee, Jawaharlal Nehru Medical College Ethical Review Committee, Institutional Review Board of the Harvard T.H. Chan School of Public Health, Population Services International Research Ethics Board, and the Ethical Review Committee of the World Health Organization. The study protocol for the pilot studies was additionally approved by the Lucknow Ethics Committee, and the Indian Council of Medical Research also approved the trial. The trial is registered at ExternalRef removed (NCT02148952). For births that were directly observed, data collectors obtained oral consent from birth attendants providing services and written consent from women or their surrogates.
