Ethics approval and consent to participate
: The Research Ethics Boards of the following organizations provided ethics approval for this study: (1) Douglas Mental Health University Institute (Quebec), (2) Vitalité Health Network (New Brunswick), (3) University of British Columbia and (4) University of Moncton (New Brunswick).
