: Because of its nature as a quality improvement/programme evaluation initiative, the University of British Columbia and Children’s & Women’s Health Centre of BC joint research ethics board deemed ethics approval unnecessary. Participants indicated consent within the online survey tool before accessing the survey.

: The first three authors are employed at and provide EIHC/KT support services at two of the organizations included in the survey and were among the multiple respondents at these sites because of their detailed knowledge of the supports in place.
