: The study was approved by Stirling University’s School of Health Sciences Ethics Committee on 19 November 2014. Redistribution of sampling across sites and an email about the study for the Association of Speech and Language Therapists in Independent Practice were approved by Chair’s Action. R&D Management Approval was received from all three participating NHS services by 31 March 2015, and Letters of Access issued. Written, signed and dated informed consent was received from all participants prior to participation.

: The signed participant consent form included item 5: ‘I understand that anonymous direct quotations from my interview / focus group may be used in study publications and presentations.’
