: Ethics approval was obtained from a University of Manchester Research Ethics Committee (Refs: 2020-9401-15954 and 2020-10067-16025). Informed, audio-recorded verbal consent was obtained from all participants prior to data collection. Before taking part, participants confirmed that they had read the Participant Information Sheet and had the opportunity to ask questions and have them answered satisfactorily. Participants also confirmed that they understood their participation was voluntary and were free to withdraw from the study at any time without giving a reason. Participants gave permission for interviews to be digitally audio-recorded, transcribed by a University of Manchester-approved transcription company and securely stored. Participant information sheets and consent forms can be made available upon request to the corresponding author.

: We obtained informed verbal consent from all participants to publish anonymised participant data (i.e. in the form of short quotations) to be used in reports and other publications, to illustrate the findings of the study. No personally identifiable data are included in this manuscript.
