The original trial adhered to the principles of the Declaration of Helsinki and received ethical approval from the Human Research Ethics Committee (Medical), University of Witwatersrand, ref: 106414, and the Research Ethics Committee, London School of Hygiene and Tropical Medicine, ref: 11644. During the original trial written informed consent was obtained from participants, who were free to withdraw at any time and were under no duress to participate.
