: The project in Guatemala received ethics approval from the institutional review boards of Brighton and Sussex Medical School (16/021/DAV), the Universidad del Valle de Guatemala (148–08-2016), and the Guatemalan MoH (16–2017). We received approval from the Guatemala MoH to use data from SIGSA and to engage with medical doctors. Local approval for community entry was sought prior to case searches in Guatemala and Idukki District. All participants provided written informed consent to participate. The research conformed to the principles of the Helsinki Declaration. The project in India received ethics approval from the Ethics Committee of Amrita Institute of Medical Sciences on 02/02/2021 (reference IEC-AIMS-2021-COMM-018). Consent from the Department of Health Government of Kerala was also obtained before commencement of the study. All participants provided written informed consent to participate or, if they were less than 18 years old, written assent with written informed consent provided by a parent or guardian. The research conformed to the principles of the Helsinki Declaration.
