: Ethics approval for this prospective mixed-methods cohort study, including all qualitative research activities was obtained from Human Research Ethics Committee of the Faculty of Health Sciences, University of Cape Town (Ref no.: 673/2019) (Medina-Marino, PI) with an institutional reliance agreement by Arizona State University (Daniels, PI) allowing the University of Cape Town to conduct IRB (protocol) reviews for both institutions. Study approval was provided by the Eastern Cape Provincial Department of Health (Ref no.: EC202010_023). All research activities were performed in accordance with the Declaration of Helsinki.
