: The institutional review boards of each center reviewed and approved the protocol; in particular the “Comitato Etico Area Vasta Toscana Centro” approved the study protocol for the Azienda Ospedaliero Universitaria Careggi in Florence, the Internal Review Board of the Rhode Island Hospital approved the study protocol in Providence and the Internal Review Board of the Rush University Medical Center approved the study protocol in Chicago.
