Ethics approval and consent to participate
: This was part of a service development project in clinical practice. The initial part of the study involved comments and feedback from our <i>cardiology patient and public involvement group</i> at our cardiology research unit. No ethics approval was needed. The project was designed and conducted in our outpatient cardiology department to improve patient experience and clinical practice. The new service included the sending of the MYMEDS questionnaire as standard practice to all patients attending the cardiology medicines optimisation outpatient clinic alongside their usual clinic invitation letter. All healthcare professionals involved had direct care responsibility for the patients attending the clinic. All participants had the option to take part by voluntarily and anonymously completing the post clinic survey which evaluated all aspects of the new outpatient service. Therefore, based on advice from our R&I department and in line with Health Research Authority guidance and its decision tools the project did not need ethics approval.
