: The study was approved by the Regional Ethics Committee, Stockholm, Sweden (Dnr 2021–03440). Before the journal review, approval was obtained from the operations manager in the included organisation. The project has received an approved ethics review without the informed consent of patients whose medical records have been reviewed. The motivation is that record review is a common method for healthcare organizations to measure patient safety and quality of care. The approach in this research project does not differ from the journal review that normally takes place in healthcare organizations.
