: The recruitment materials used in this study were approved for use in the FAST study by Scotland A Research Ethics Committee, 11/AL/0311. Informed written consent has been obtained from all FAST study participants. Specific informed consent was considered unnecessary for this sub-study as there was minimal additional burden to individuals and knowledge of supplementary information materials may have confounded the evaluation.
