: The protocol, informed consent, and data collection forms for the primary HIPSS study were reviewed and approved by the Biomedical Research Ethics Committee of the University of KwaZulu-Natal (Reference number BF269/13), the KwaZulu-Natal Provincial Department of Health (HRKM 08/14), and the Associate Director of Science of the Center for Global Health (CGH) at the United States Centers for Disease Control and Prevention (CDC) in Atlanta, United States of America (CGH 2014–080).
